Using your invoice for insurance reimbursement
Many private insurance plans reimburse the Freedom Leg under HCPCS code L2136. Coverage depends on your plan. We cannot bill insurers directly: you buy the Freedom Leg direct from us, then submit your invoice to your plan for reimbursement.
What you need
- Your invoice. We email it to you as a PDF after your purchase. It carries our supplier name, Tax ID, NPI, the HCPCS code (L2136), the date and the amount paid.
- A prescription or letter of medical necessity from your doctor that names the Freedom Leg brace and HCPCS code L2136.
- Your plan's member claim form.
How to submit your invoice to your insurance
- Ask your doctor for a prescription (or letter of medical necessity) for the Freedom Leg that names HCPCS code L2136 and your diagnosis.
- Get your plan's member claim form. Ask for the form for "out-of-network" or "DME / orthotic" reimbursement; most plans have it on their website or app.
- Attach the invoice and the prescription. The invoice already carries the supplier name, Tax ID, NPI, HCPCS code, date and amount paid.
- Submit to your plan and keep a copy.
- If the plan asks questions, the numbers they need are in the insurance panel on page 1 of the invoice.
Good to know
- We cannot bill your insurer directly: you buy direct and submit.
- Not covered by Medicare or Medicaid. Some supplemental or Medicare Advantage plans may reimburse; ask them about L2136.
- HSA and FSA cards can be used for the purchase.
- If your plan only reimburses purchases made through an in-network supplier, call us at (888) 816-8127 and we will try to help find one.
- Your doctor's office may also be able to order the brace and handle the paperwork.
